You have newly joined a three-member committee on your unit responsible for completing the peer evaluations of your colleagues.  The first two evaluations went smoothly.  The committee followed the approved evaluation criteria and successfully reached agreements on the initial evaluations.  Today, the committee is meeting to work on the yearly peer evaluation of Jacob, one of your coworkers.  Jacob has been employed in your hospital for five years as an RN and has worked on your unit for the past year.  When you bring out the evaluation form, Lisa, another committee member, slams down her fist on the conference table.
