Where do you see the field of developmental psychopathology going?

Discussion 2(Learning Exercise 6: Pediatric Psychology Case Example)

Luna is a 15 year old female who is brought to the diabetes clinic by her two dads for her check-up with her endocrinologist. Luna’s blood sugars have consistently been high for the last 2 months and it is clear that she is not checking them prior to every meal and snack as she should. Luna also reports to her doctor that she is not taking her insulin as needed to control her higher blood sugar. You, the pediatric psychologist, are brought in to discuss medical non-compliance. In speaking with Luna privately, she reports embarrassment of checking her blood and giving herself insulin shots in view of her peer. Further she reports sneaking sugary food because she “wants to feel like her friends” even though she is aware of the consequences. Mood wise, Luna reports feeling sad and hopeless most of the day for the last two weeks, notes increased levels of fatigue, worse sleep, poor energy, and passive suicidal ideation. Luna denies a current suicidal plan or intent, but does note that she frequently thinks “what if I wasn’t here.” When speaking privately with her dads, both report increased fighting and tension surrounding her diabetes care. They also note that her grades have worsened and she doesn’t seem to want to spend time with her friends any more.

As her pediatric psychologist, ask yourself the following questions:

  1. What disorder(s) come to mind when reading Luna’s case? What symptoms align with this diagnosis?
  2. Do you have any safety concerns for Luna? If so, what is your plan to address them?
  3. What solution-focused techniques might you suggest to support Luna’s medical compliance? How/if do they involve Luna, her school and/or her dads?
