GCC Delegation Rights of Nursing Worksheet

Worksheet of the first assignment is attached,
“Scope of practice assignment”. it needs to be filled out. I have also attached the 5 rights of delegation for reference.
The second part is a group assignment, I am assigned to do the RN portion. I have attached a template. Here is my group’s case scenario:
- 75 y/o overweight middle eastern male
- History of hypertension, smoking 1.5 pack a day for 40 years, hyperlipidemia
- At telemetry unit 2 days post cerebrovascular accident (stroke)
- Headache 5/10
- Left sided weakness, numbness, tingling, difficulty swallowing, slight shortness of breath, slurred speech
- Vitals taken
- On IV Heparin to break down the clots and to prevent formation or clot
- Pt has Acetaminophen (Tylenol) order for pain
- Imaging and labs performed
- Standing order for Warfarin 10mg tablet, once a day, by mouth to prevent blood clots and heart attack
