Community Assessment & Planning (CAP) Project
The modified windshield survey is a composite of objective and subjective data that help define
a community, its trends, stability, and changes that will affect the health of the population. The
windshield survey must include the elements listed below. Use this format for reporting your
findings by including the categories as headings. The scope and depth of the assessment will be
important in the overall grading of the assignment.
Although you are required to actually conduct a common and iconic éndshield survey,9ou
also must acquire further objective evidence about the health status of the community.
Therefore, you need to search relevant databases from local, county, and state entities, such as
a county health department. In addition, census bureaus, town administrations, city halls,
libraries, chambers of commerce, fire and police departments, park districts, town Facebook
pages, city newspapers, are all resources that can contribute to a more thorough assessment of
the community.
Required Categories of Evidence for the Assessment Section
Oak Brook Community
Introduction: use bullet point to give an introduction about Oak brook, IL.
Boundaries: Identify the boundaries (streets, highways, river, landmarks, etc) of the
community. Include a map, highlight the area that you assessed and state what lies
north, south, east and west of your area. Does the neighborhood have an identity or a
name that is displayed or unofficial?
Demographics: Note the following data about the population: Age, sex/gender,
ethnicities/races, incomes, education, employment, home ownership, religions.
Housing/Economics: How old are the houses and what are the construction materials?
How well are the homes maintained? What kinds of multiple family dwellings exist?
Are there vacant lots? Is this an established area or is there a lot of new construction?
Are there for-sale signs? What does the housing indicate about the income level of the
residents? What types of industries are in the area? Who are the major employers?

Make sure references are from 2019-present. Each slide should include bullet points not paragraphs.
Include pictures related to each topic. You can use Zillow if you want for housing part. Give adequate
summary for each section on a separate document.
